Have not been accepted to Mobius PVE ingame

Hi folks, I posted to the introductions forum several days ago and I have requested Mobius PVE in game group.
My request has not been answered and apparently every couple days the the request needs to be renewed as it says there is no pending request.
Am I doing something wrong or is there something I missed?

form?

I've just checked the reg sheet for Mobius PvE and can't see cmdr UthervL, i can see the request in game though. oncee i have an entry on the
registration sheet i'll accept you, it will be tomorrow morning now at the earliest.

I did fill out the form from a tablet maybe that's the problem or I just screwed it up it seems.
Thanks for the fast reply I have filled out the proper link from the pc I will be using
